CORRECTION OF THE INVERTED NIPPLE

The inversion of nipple presents a functional problem in nursing and also a psychological problem owing to the abnormal appearance. Numerous methods have been reported to correct this condition. We devised a simple procedure for correction of inverted nipples. The nipple is pulled out by traction sutures with 4-0 nylon. Traction is maintained throughout the following procedure. Two diamond-shaped incisions are marked at 2 and 10 o'clock positions. The short diagonal lies on the borderline between the nipple and areola. Incisions are made and small deepithelized dermal flaps are raised. The fibrous tissue beneath the inverted nipples are released by blunt dissection. The dissection is parallel to lactiferous ducts to avoid damage. Each of the flaps is turned down through the tunnel and sutured each other to give bulkiness to the nipple. A 4-0 nylon buried purse-string suture is placed at the base of the nipple. The remaining wound is closed in two layers. The newly everted nipple is maintained by a sombrero splint in place with sutures. From March of 1992 to March of 1996, 24 nipples(16 patients) are corrected by this method. The eversion obtained by our method is excellent and persistent without any recurrence. No major complications are occurred.
