Traditional transcutaneous blepharoplasty has had shortcomes of external scar, long recovery time and lower lid retraction. Authors have performed transconjunctival blepharoplasty of 95 cases in patients, who visited the clinic with the complaint of excessive fat of lower eyelids from 1995 to the present and followed up these patients for 2-24 months. We could avoid the external scar and lower lid retraction and obtain the early recovery and decreased operative time. By using the radiosurgical current (Elman surgitra○R , 72 cases) and CO2 laser (Ultrapulse○R , coherent, 23 cases)as incisional device, we obtained the decreased postoperative bleeding and edema and relieved the postoperative pain. Even though patients complained the undercorrection (2 cases) and increased skin wrinkling (2 cases) resulting from complications, We could correct the undercorrection through the secondary operation and fine wrinkles through the laser resurfacing and excessive skin through the minimal skin excision. There were no cases of ectropion, inferior oblique muscle palsy and overcorrection.
Our department has performed the transconjunctival blepharoplasty to those who had baggy eyelids without markedly fine wrinkles. Since we achieved a satisfactory result aesthetically through the transconjunctival blepharoplasty on patients who shows proper indications, we report the indications, operative technique, the adjunctive technique, and complications in addition to the review of literatures.
