Previously, a method of negative-pressure-assisted dispase splitting into various thickneses and sizes of surgically obtained human or animal skins into the epidermal and dermal layers was invented. Good penetration of the enzyme into the skin tissue was obtanied by applying negative pressure in a liposuction apparatus and then rapidly separating the skin tissue to the room air pressure when the pressure pushed the enzyme solution deep into the dremal tissue.
The procedure was repeated every 3 minutes under 750mmHg negative pressure assisted dispase(100IU/ml). The earliest time for separation of viable epidermal sheet was measured. As results, 750 mmHg negative pressure, 7 times of repeats, 37.5°C 25 mins incubateion times were the factors for the earliest separation of the eqidermal sheet. Then the author suggested that this was an acceptable method of treatment of tattoos and nevus. No donor site morbidity and short wound healing time were advantage of this method. Based on above results, the author experienced the separated skin graft in 18 cases, follow up ragne from 6 months to 18 months. among 18 patients, 15 cases were in artificial tattoo and remaining 3 cases were in scar. The most common site of the lesion was forearm(15 cases). Postoperative analysis of separated skin graft was based on hypertrophic scar formation, amount of secondary contracture, patient's satisfaction. Postoperative results of separated skin graft were similar to that of split thickness skin graft. Two cases were failed by infection(delayed healing, excessive hypertrophic scar formation). Sixteen cases were satified to postoperative results.
