Acute Cerebral Infarction and Visual Loss after Autologous Fat Injection into the Face

Autologous fat injection for soft tissue augmentation in the face is a well-known technique in aesthetic surgery. And It is claimed to be a safe procedure. However, there are several case reports in the literature where patients have suffered from acute cerebral infarction and visual loss after such interventions. We report another patient who developed an embolic infarct and ocular fat embolism after autologous fat injection. We assume that fragment of fat was injected into one of the peripheral branches of the ophthalmic artery. Under injection pressure of the syringe, the fat was likely forced retrograde into the ophthalmic and internal carotid arteries with subsequent distal movement of the emboli into the ocular and cerebral arteries. At present, MRI-T2 weighted image is the most sensitive imaging technique for diagnosing cerebral fat embolism, and correlates well with the clinical severity of brain injury.
Treatment of these complications is only supportive care. To minimize the risk of such a major complication, fat injection should be performed slowly, with the lowest possible force.
One should use larger, blunt cannulas and epinephrine at injection site and avoid the injection of large boluses volume and fat injection into pretraumatized soft tissue.
