Clinical Experience in Com-bined Treatment Using Mechanical Dermabrasion and Laserbrasion for Facial Scars

Many methods have been used to treat common facial scars such as acne scars, pockmarks and posttraumatic scars. The single treatment of mechanical dermabrasion, laserbrasion and chemical peel can have less than satisfactory outcomes and can result in a few complications. To minimize postoperative compli-ca-tions and acquire more effective outcomes, the authors selected a combined treatment using mechanical dermabrasion and laserbrasion except chemical peel due to the difficulty of ap-plication on Asian patients. The authors treated 7 cases(4 males and 3 females) including pockmarks(n=4), acne scars(n=2), post-traumatic scar(n=1) with combined mechanical dermabrasion and laserbrasion between 1998 and 2004. The patients ranged in ages from 19 to 56 years at the time of surgery, with a mean age of 36 years. Our operative technique was to use a diamond impreg-nated burr, performing mechanical dermabrasion roughly, and then, to use CO2 laser with a computerized pattern generator, performing one pass CO2 laserbrasion in whole face. Finally we switched to a 3-mm collimated handpiece to treat localized areas of narrow and deep scars. Epithelization was completed within 7 to 10 days after operation and there was improvement of scar and no hypopigmentation or hyperpigmentation except one case, in which the hypertrophic scar was developed but it was also im-proved after additional laserbrasion. From 7 to 38 months(mean, 31 months) postoperatively, the results were evaluated by the ordinary scale method using preoperative and postoperative photographs and questionnaires about patient's thought of out-come. By the ordinary scale method using preoperative and post-operative photographs, “excellent” aesthetic results were noted. By assessment of satisification using questionnaires, loss of scar was “excellent” and recovery of normal skin color and recom-men-dation to other patient were “good". This method was more delicate, less bleeding, less painful and more collagen regenera-tion than only mechanical dermabrasion and took a shorter operation time than only laserbrasion. This study found that combined treatment using mechanical dermabrasion and laser-brasion is one among the effective methods of the treatment of facial scars.
