Mesocephalic oriental face is short and wide, and in relation with a flat nose prominence of zygoma will make the face flatter, older and appear musculinic. Thus many people have prominet zygoma want to reduced their zygoma for making their face more cheerful, youthful and aesthetic. Also there is some superstitious belief like a fortune of widow in Korea.
Various methods such as chiseling or burring of the zygoma body and arch, segmental osteotomy and repositioning of the arch via a bicoronal approach were frequently used for zygoma reduction, but the letter was usually less effective in reducing the wide face and the former involved an extensive operation and left a long visible scar. We thought these methods were less than ideal as aesthetic procedures. A simple and yet effective method of reducing the prominet zygoma was needed.
Reduction of the prominet zygoma was performed by shaving the zygoma body and inward displacement of the zygomatic arch after two point fractruring; green stick fracture anteriorly and complete osteotomy posteriorly, via a small prearuicular and upper buccal sulcus incision. We obtained satisfactory results using this relatively simple procedure. The advantages of our technique are; 1) small skin incision and resultant inconspicuous scar 2) simple but effective 3) no needs of bone fixation.
