Personal Experiences in the Clinical Application of Dermofat Graft

The dermofat graft has been used clinically in the  various parts of the patient in the plastic and reconstructive surgery. It has been used in nasal augmentation, particularly secondary rhinoplasty, in addition to the primary case, soft tissue augmentation following  surgery and correction for depression scar, etc. Various other materials have been used for many years in these applications. The injection of foreign materials has been carried out, in spite of some risks. However, dermofat is an autologous tissue of the patient's own body, and has some advantages.
Over the last 3 years, the authors have been studying 38 cases of patients with aesthetic or soft tissue problems. Of these, 22 patients were augmented with dermofat grafts, 10 for augmentation of the soft tissue and the remainder for depression scars.
Dermofat graft has been
1. Used for the reconstruction of the adipose cushion.
2. Well tolerated and minimally absorbed.
3. Used for the maintenance of the semi-soft tissue consistency although they undergo a partial fatty necrosis and fibrotic change.
4. Found not to cause inconveniences of the capsule formation compared to the rigid prosthesis.
In our experiences, dermofat grafting has wide clinical applications, especially small-to-medium sized soft tissue defect. Careful handling and fixation may reduce the absorption rate. This enables minimal overcorrection. It will be proper to augment soft tissue depression combined with scar revision. It is to be suitable for primary rhinoplasty as well as secondary case. Its complications are rare and easily controllable.
