In case of the upper blepharoplasty, it is necessary to specify preoperatively the size of the fold height and the shape of the double eyelid according to the patient's desire. One must predetermine the level of the lower skin incision as well as the amount of skin to be excised to obtain a desirable double eyelid.
The authors devised a method to be used for determining the amount of skin excision at the preoperative design in the upper blepharoplasty. We assumed that 20 mm of skin from the lid margin to the lower border of eyebrow at the midpupillary level should be left to achieve good results. We regarded the skin amount over 20 mm as the amount of the extra-skin and decided the half of that to be removed. We modified the excision amount according to the state of individual skin laxity case by case. This technique was performed in 48 consecutive cases (96 upper lids) for one year period. The overall results obtained were satisfactory and this method was easy, accurate, and predictable
