The blepharoptosis is the disease which is commonly caused by weakness of the levator palpebrae superioris or its aponeurosis. Therefore abnormally low level of the upper eyelid during straight-ahead gaze interferes with the visual field. From the January 1993 to December 2000, authors had treated 44 blepharoptosis patients. The follow-up period of patients ranged from 1 month to 6 years with a mean of 41 months. Operative procedures were chosen according to the patient's levator function and ptosis amount. Levator resection in the case with levator function 4-8mm, ptosis amount 2-4 mm was 17 cases. Frontalis transfer in the case with levator function 1-6mm, ptosis amount 1-5 mm was 27 cases. The result were evaluated with the average 41 months of follow up and we compared the result of the levator resection with frontalis muscle transfer. The preoperative average amount of ptosis is about 2.6 mm in the case with levator resection, about 4.1 mm in case with frontalis muscle transfer. The postoperative average amount of ptosis is about 1.5mm in the case with levator resection, about 2.1mm in case with frontalis muscle transfer. Authors' frontalis-orbicularis oculi advancement technique include the orbicularis oculi muscle in the distal end of the frontalis myofascial flap but doesn't made medial and lateral vertical incision of orbicularis oculi muscle. Authors' frontalis-orbicularis oculi advancement technique could be applied to the case with levator function 1-6 mm, ptosis amount 2-4 mm, and the preoperative average amount of ptosis is about 4.0 mm, postoperative average amount of ptosis is about 2.0 mm.
