In most cases, primary cleft lip repair has contributed to the ability to achieve a normal configuration of the repaired lip. However, some problems in the correction of the cleft appearance after surgical repair remain to be solved. There are nostril sill depression, vermilial notching, and philtral column and dimple formation, which should not be overlooked in correction of secondary cleft lip nose deformities. It is clear that problems in the repaired cleft lip are due to a defect in the soft tissue and a defect in the supporting bony framework and the method that may be used for solving these problems is based on supplementing the soft tissue and bony framework. The supplementation of bony framework has many drawbacks compared with the gains, but supplementation of the soft tissue is easy. If the soft tissue supplementation could be obtained without tissue sacrifice, it would be ideal. 
 We used vertical scar flap for correction of nostril sill depression, absence of philtral column and dimple, and whistle deformity because the scar tissue is less limited in amount. If we wanted to correct nostril sill depression, we used a superiorly based vertical scar flap. If we wanted to correct whistle deformity and to form philtral column, then we used a inferiorly based vertical scar flap. The results were satisfactory and the patients were also satisfied. This method has two advantages.      One is that correction of residual deformities and scar revision can be done by one single operation, and the other is that there is no sacrifice of donor tissue.
