Mandibular angle reduction could be performed by external or internal approach. Nowadays, internal approach through oral cavity is more preferable because of some problems such as inacceptable scar and facial nerve injury in external approach. But internal approach method has also significant complications like subcondylar fracture and asymmetry. So mandibular angle reduction by combination of internal and external approach method with 3mm incision below the chin line was tried to decrease the incidence of complications and reduce postoperative morbidity and operating time. From February 1997 to December 1999, mandibular angle were reduced in 72 patients by this method. The patient's purpose of surgery was all cosmetic reason and reductions were done bilaterally in all patient. Mandibular angle and designed ostectomy line was exposed through 3cm intraoral incision and then reciprocating saw was placed to intraoral exposed angle area with the connector of saw passed to outside through the 3mm skin incision made with no. 11 blade. After connection to the handpiece, angle ostectomy was performed easily under the direct vision. In 72 patient, complications were 1 hematoma and 2 transient hypoesthesia on lower lip, but subcondylar fracture or asymmetry was not happened. In comparing with external approach, advantages of this method are more acceptable scar, very low possibility of facial nerve injury and comparing with internal approach, easy and exact ostectomy without possibility of subcondylar fracture, short operation time and less postoperative discomfort. Resultant scars on chin due to external incision were all acceptable after postoperative 2 months, there was no one showing hypertrophic appearance. Mean operation time was about 2 hours including anesthesia and surgical preparing. 3mm external approach under the chin is thought one of the good method for mandibular angle reduction with low possibility of significant complication.
