Male type baldness resulting from androgne usually causes in hairlessness on the vertex, midscalp, and frontal area. Patients want to correct the baldness mainly due to cosmetic problems. Traditionally, the methods of treating this problem were classified as hair graft, scalp transposition flap and scalp reduction. Since the pioneering report of tissue expansion by Newman and Radovan, their technique has been widely practiced by many plastic surgeons.
With expansion-assisted advancement, the frontal hair line can not be corrected with hair-bearing scalp. In our experiece, we have noted that scalp expansion, with Z-plasty, successfully corrected frontal hairline without delay in the procedure and covered vertex and crown with hair-bearing scalp.
We treated 5 cases of male type baldness during the recent 1 year. In 2 cases, we obtained excellent results from expansion-assisted advancement flap with Z-plasty for the frontal hair line. Two cases of patients forelocks were treasted with excellent result using the advancement flap only. In one case, we experienced exposure of expander and thereafter had partial failure in raising of the expander-assisted tempro-parieto-occipital flap.
