Recent improvement of life quality, change in self confidence and increased the social activities of women have forced the number of the female aesthetic or reconstructive operations to increase in their effort to achieve attractive external appearence. Especially women receive aesthetic breast surgery in high incidence than in male because the breast is the sexual symbol of female. In Korean society many women think their breast is too small and various surgical procedures are performed to augment the volume of breasts. In general augmentation mammoplasty with silicone or saline implant is mainly perfomed surgical procedure and its postoperative result is also satisfactory, however serious complications such as capsular contracture or implant rupture still remain problems that occur more frequently compare to other aesthetic surgery. Also the management of these complication is very difficult and there has been no agreement in managing the capsule. Then the methods we adopted were changing the augmentation plane (subglandular to submuscular), removing of contracted capsule by partial circumferential capsulectomy techinque, using Ether solution for removal of leaked silicone gel form the implant, changing smooth type implant to textured type and doing mastopexy if need. And injection of foreign body such as paraffin or silicone materials in breast tissue to increase the breast volume taken over by unscrupulous practitions in Korea previously and even today. And the complete removal of foreign body from the breast tissue is very difficult. Also differential diagnosis from breast cancer is nearly impossible. We solved these problems by subcutaneous mastectomy for complete removal of foreign body and subpectoral augmentation with textured saline filled mammary implant. Here we present our experience in 19 cases of reoperation to manage the complications after augmentation mammoplasty that showed satisfactory result and had longterm follow up was possible from Jan. 1989 to Sep. 1999 in Severence Hospital and wish to discuss the proper management for these complications.
