Cryptotia is a uncommon malformation of the auricle in which the upper pole of the helix is invaginated under a fold of temporal skin. Pulling the ear serves to draw out the auricle, but it retracts immediately after letting go. The auriculocephalic sulcus is obliterated. In addition, there is often a deformity of the antihelix, especially its superior crus. There are various available methods that can be used for the surgical correction of cryptotia, but their main purposes are the coverage of skin raw surface result from elevation of invaginated ear helix and the correction of deformed cartilage. For the correction of cryptotia, the authors carried modified Onizuka’s method to avoid the handicaps of Onizuka’s method i.e. the disturbance of hairline and the deficient auriculocephalic sulcus. Our method is modified Onizuka's Z-plasty in which another Z-plasty is added to the retroauricular flap end. This method is used to cover the skin defect and cartilage plasty is employed by Onizuka's method that cartilage graft form cavum concha served as a splint for prevention of recurrence of cartilage deformities. We have treated 4 patients using this technique and the results were very satisfactory to the patient. The correction of cryptotia by double Z-plasty is good choice the treatment of cryptotia i.e. not tension and natural apperance.
