Ptosis of the upper eyelid is one of the most common deformities of the eye region. It is very difficult to have satisfactory postoperative result for the severe blepharoptosis. The frontalis muscle suspension with fascia or direct transplantation of frontalis muscle to the tarsal plate had been used by many authors in the treatment of severe blepharoptosis but these procedures have some inherent shortcomings such as morbidity of the donor site in the fascia suspension procedure and loss of wrinkle, bleeding, difficult dissection, depression on the forehead and injury to the supraorbital neurovascular bundle in the frontalis muscle flap techinque. Orbicularis oculi muscle flap has some advantages such as being a simple technique with a good operative field , single incision and less complication. But it results in some undercorrection or droop of the upper eyelid over a period of time in cases with absent or very weak levator function. The author found that some fibers of frontalis muscle and its fascia extend downward beyond the supraorbital rim and interlace at the eyebrow region with the orbital portion of the orbicularis oculi muscle. The interlaced muscle fibers of the frontalis and the orbicularis oculi muscle at the eyebrow region is relatively mobile upon the periosteum due to the areolar tissue beneath the muscle layer.
  Authors had experienced 33 cases of blepharoptosis from March, 1991 to July, 1994. The operative procedures were chosen according to the patient's levator function: Frontalis muscle falp with or without frontalis myofascia in 15 cases who have levator fuction below 2mm, orbicularis muscle flap with or without frontalis myofascia in 18 cases who have levator function between 2-5mm. We would like to say that we can reduce the complication of frontalis muscle flap and prevent the recurrence or unsatisfactory results of orbicularis oculi muscle falp. So, we can achieve more satisfactory results.
