With the aging process, the anatomic changes of the lower eyelids and malar area show progressive skeletonization and orbitalization. In the orbital area, palpebral bag redundant lower eyelid skin are developed as a result of the weakening of the orbital septum and atrophying of the orbicularis oculi muscle. These changes, including the nasojugal and the palpebromalar fold, can be seen obviously by the end of the fourth decade. In the conventional lower blepharoplasty procedure, the excessive lower eyelid fat is removed after elevating the orbicularis oculi muscle from the subciliary line extending into the infraorbital rim. And then the skin flap or the skin-muscle flap is re-draped and trimmed. However, this conventional lower blepharoplasty technique cannot correct the problems of wide orbit, tear trough and the skeletonization of lower orbiral rim.
  We, the authors, therefore have used the lower blepharoplasty with fat sliding technique which was reported by Hamra in 1994 to solve these problems.
This study includes 52 patients, treated at the Jung-Lee's Clinics of Plastic and Reconstructive Surgery and at the Seoul Red Cross Hospital Between November, 1996 and March, 1998. The postoperative results of lower blepharoplasty with fat sliding technique are functionally and cosmetically excellent.
  The advantages of the lower blepharoplasty with fat sliding techniques are as follows;
  (1) Fat sliding technique can be applied in the cases of secondary lower blephroplasty.
  (2) The nasojugal and the palpebromalar folds can be corrected effectively. 
  (3) The malar bag can be corrected which cannot be corrected using the conventional lower                  blepharoplasty technique.
  (4) Midfacial ptosis can be corrected by stretching the lateral side of orbicularis oculi muscle.
