Many Asian have broad nasal tip and minimal tip projection because their nose show convex lateral crura and wide angle of domal definition which consists of medial crura and lateral crura. So, nasal tip plasty is essential in Asian rhinoplasty process. In nasal tip plasty, there are three major subgroups-     sculpturing, suturing, tip graft. Generally, sculpturing and suturing technique are not enough to get define nasal tip. In tip graft, L-shape silicone, ear cartilage and septal cartilage have been used. However, L-shape silicone has so many problem-tip erosion, infection and so on- and ear cartilage graft tends to be deformed with age. Therefore, septal cartilage has been prevalent for tip graft in Caucasian nose. We did 407-tip plasty with septal cartilage from July 1994 to February 1998 in 702 rhinoplasty patients. It takes about 3 minutes to obtain the septal cartilage and it is in the same operative field. For the follow up period of more than 4 years, only minimal complications were observed. No absorption, no skin erosion and no deformity occurred. The patients' satisfaction was very high. Moreover, this method has showed much merit(Table I). Therefore, we conclude that the septal cartilage should be the first choice in Asian tip plasty.
