The charcateristics of the aging face can be divided into three catagories; skin and soft tissue laxity (gravitational factor), elevator muscular compensatory hyperactivity and prolonged depressor muscle activity. This dynamic but opposing muscle activity eventually produces hypertrophy and a series of wrinkling and folds in the glabellar, orbital, perioral, and mental areas. Gravitational forces, in the meantime, produce malposition of the facial skin and the fat pads in each of these areas. Therefore, principle of correction of the aging face is to make natural harmonious balance of all repositioned anatomic areas of the aging human face. We experienced 49 rhytidectomy cases, from May, 1995 to March, 1998. All of these 49 cases were operated by using the author's new methods, endoscopic forehead lift without muscle resection and facial lift with skin stretching device. The author's forehead lift has the following advantages:
1. Minimal incision
2. No limitation of facial expression without myotomies and, or, neurectomies
3. Minimized swelling, edema with limited dissection and suction drainage
4. Maximizing lifting effects with two horizontal periosteal release
5. Easily combining with mid, or lower face lift.
6. Natural harmonious facial structure balance without exaggerated mongoloid slant
Author's facial lift using skin stretching device has the following advantages.
1. Easy operative procedure
2. No need for wide dissection
3. No blockage of blood supply
4. Minimizing complications
5. Decrease of postoperative swelling, edema, ecchymosis and pain
We concluded that our method of facial lift is a procedure for achieving a harmonious balance of all repositioned anatomic areas of the aging human face.
